Community Integration Program Quality and Patient Outcomes
January 1, 2008 through December 31, 2008

Adult Day Program

110 patients were discharged from Adult Day Program with the following diagnoses:
                        Stroke – 50%
                        Brain Injury – 22%
                        Spinal Cord Injury – 21%
                        Other – 7%

  • 95% of patients were better able to participate in community activities at discharge than they were upon admission to the program.
  • 100% of patients who had a discharge plan to return to work, school, or home management or had a plan to enter a VR program have reported 90 days after discharge that they have returned to work, school, home management or entered into a VR program.
  • 97% of patients were satisfied or very satisfied with the results they achieved in their program.

"We didn't meet anyone who wasn't gracious, helpful and knowledgeable."

"All family members were kept informed."

Adult Neuro Outpatient Rehabilitation Program

170 patients were discharged from the Adult Neuro Outpatient Rehabilitation Program with the following diagnoses:
                        Stroke – 29%
                        Spinal Cord Injury – 26%
                        Brain Injury – 16%
                        Orthopedic – 14%
                        Neurology – 5%
                        Other – 10%

  • 93% of patient goals were achieved before discharge.
  • 92% of patients reported that their ability to care for their daily needs has improved or stayed the same 90 days after discharge from the program.
  • 100% of patients seen for an evaluation only reported that the information and recommendations received during their evaluation was useful and met their needs.

“It is not very often that one is able to find a place that truly strives to give the best care and takes great pride in their work, but this is the feeling that I received whenever I walked through the front doors at The Rehab Institute.”

“We could not be more pleased with the services we received.”

Pediatric/Adolescent Program

165 patients were discharged from the Pediatric/Adolescent Program with the following diagnoses:
                        Brain Injury – 45%
                        Cerebral Palsy – 25%
                        Stroke – 6%
                        Seizure disorder – 6%
                        Spinal Cord Injury – 2%
                        Other – 16%

  • 100% of patients who were appropriate for school placement were successfully transitioned to school.
  • 98% of patients reported that they have played with other children their age and/or identified alternative age-appropriate social activities 90 days after discharge.
  • 100% of patients reported that they felt ready to return to school and social activities upon discharge from the program.
  • 100% of parents reported being satisfied or very satisfied with the outcomes their child achieved in their program.
  • 100% of patients reported that they felt involved in the decisions made about their care and that they understood their therapists’ explanations.

"I am always pleased with everyone I come into contact with.  Everyone always goes above and beyond their duties. I am so glad The Rehabilitation Institute is there." (Pediatric parent)

"Rehab was awesome." (Adolescent patient)

Driving Evaluation and Training Program

104 clients were discharged from the Driving Program.
  • 68% of clients who were evaluated were recommended for driver training.
  • 39% of clients approved for driving had adaptive equipment recommended.
  • 97% of clients reached for follow-up have driven 90 days without an accident.
  • 97% of clients reached for follow-up have driven 90 days without a moving violation.
  • 94% of clients were satisfied overall with services received.
