Personal Health Care Profile

Creating a personal health care profile can be helpful to you, your family and your health care providers.    A complete profile includes your emergency contact information, hospital preferences, your primary physician(s), insurance providers and a list of any medications you are on and/or any allergies or special circumstances that would be helpful for medical professionals to know in the event you are not able to communicate the information directly.  This information should be shared with all physicians you see.  It can help improve the quality of care you receive; reduce or eliminate duplicate tests; and may help you receive more appropriate care in an emergency.  It also helps you to take an even more active role in your health care.

We recommend that you also attach copies of Advanced Directives, Living Wills and/or Durable Power of Attorney for healthcare to your Health Care Profile.
