• The average age of patients discharged was 15.75 years.
• The average range of patients admitted was 12 to 21 years.
• The patients had the following diagnoses:
  • Brain Injury - 30%
  • Spinal Cord Injury – 15%
  • Stroke - 12%
  • Cerebral Palsy - 21%
  • Other - 28%
• Day treatment patients participated in an average of 19.1 program days.
• Outpatients averaged 11.4 therapy hours.
• 94% of age appropriate patients completed transition to school.
• 100% of patients remained in the same or better school placement 90 days after discharge.
• 100% of adolescent patients felt ready to return to school and activities with friends.
• 100% of parents and patients were satisfied with the results their child achieved.
• There were no patients who unexpectedly returned to acute care.

• The average range of patients admitted was 21 months to 12 years.
• Patients had the following diagnoses:
  • Head Injury - 30%
  • Cerebral Palsy - 24%
  • Neurology - 7%
  • Stroke - 6%
  • Spinal Cord Injury - 5%
  • Other - 28%
• Day treatment patients participated an average of 19.4 program days.
• Outpatients averaged 32 therapy hours.
• 97% (60 of 62) of age appropriate patients completed transition to school.
• 100% of parents were satisfied with the results their child achieved.
• 100% of patients were discharged home and 100% of those reached were still living at home 90 days later.
• 100% of patients under age 4 gave their program a “thumbs up” when asked if they liked their experience.
• There were no patients who unexpectedly returned to acute care.

• The average age of patients discharged was 58 years.
• The average range of patients admitted was 21 to 81 years.
• Day Program patient diagnoses:
  • Stroke - 51%
  • Brain Injury - 31%
  • Spinal Cord Injury - 12%
  • Other -  6%
• Patients participated in an average of 19 program days.
• 96% (69 of 72) of Day Program patients were satisfied overall with the results they achieved.
• 100% of patients were discharged home and 99% of those reached for follow-up were still living at home 90 days after discharge.
• 100% of patients who were appropriate, had returned to work, school or home management 90 days after discharge.
• 3 patients returned to acute care during their program.

• The patients in the program had the following diagnoses:
  • Stroke -  38%

  • Spinal Cord Injury - 18%
  • Orthopedic - 10%
  • Other - 6%
• Patients participated in an average of 10.7 hours of therapy
• 99% (76 of 77) of patients' abilities to care for daily needs such as bathing & dressing had improved 90 days post discharge.
• 100% of patients were discharged home and 99% of those reached for follow-up were still living at home 90 days after discharge.
• 100% of patients surveyed felt their evaluation recommendations were helpful.
